Impact of immunonutrition on inflammatory and nutritional biomarkers in perioperative HIPEC patients

Objetivo: Avaliar o comportamento da proteína C-reativa (PCR) e da albumina em pacientes submetidos à cirurgia citorredutora com quimioterapia intraperitoneal hipertérmica (HIPEC) sob protocolo de imunonutrição. Métodos: Coorte observacional retrospectiva com nove pacientes com carcinomatose peritoneal atendidos entre 2021 e 2024. Os marcadores foram avaliados em cinco momentos perioperatórios. A normalidade foi testada por Shapiro-Wilk. Utilizaram-se correlação de Spearman e testes não paramétricos. Resultados: Observou-se pico de PCR 48h após HIPEC e redução progressiva da albumina no mesmo período. A variação pré-operatória da PCR apresentou correlação positiva com número de complicações (R²=0,542; p=0,156). O IMC na alta correlacionou-se negativamente com complicações (r=-0,66). Conclusão: A HIPEC impõe estresse metabólico intenso evidenciado por alterações inflamatórias e nutricionais. A monitorização dinâmica desses biomarcadores pode auxiliar na identificação de pacientes com maior risco de complicações.
